The Navigation Model
Over the last 4 years we have been navigating people with Multiple and Complex Needs around the system in Newcastle and Gateshead, helping them access the support services they need to lead a more fulfilling life.
Because of their Multiple and Complex Needs, the people we support often faced many barriers to the support they deserve, with services seeing them as ‘hard to reach’ or ‘too difficult’.
Our experience of navigating people over the past 4 years of our programme has developed our understanding of what makes a good multiple complex needs worker and going forward we will continue to implement these 5 key skills;
- engaging people through validation
- building motivation and hope
- working in collaboration
- building trusting relationships
- and establishing realistic expectations
We will continue to offer our navigation model to some of the people we support over the next 2 years as we know it works for some! This navigation support includes a ‘refer and accompany’ approach to enable people to access the services they need to meet their needs. The navigation approach is not time-limited and has an open approach to the focus on support planning and goal setting.
